Everyone coming to SC4’s campus is required to go through one of the on-campus screening stations. This is in addition to the requirement to wear a face covering correctly and practice social distancing whenever possible.
To help the screening process go as quickly and smoothly as possible, please complete the online COVID-19 Self-Assessment Form each day before coming to campus. If you’re cleared to come to campus for that day, you’ll see a green check mark on your response page. Bring your phone with the response page, a screenshot of that page, or a paper copy of the page showing the check mark and the date. You will also receive a confirmation email that includes the check mark and the date that you can use. You’ll show it to the campus screener during the screening process.

Fine Arts Building, north entrance
- 7 a.m. to 6:30 p.m., Monday
- 7 a.m. to 5 p.m., Tuesday
- 7 a.m. to 6:30 p.m., Wednesday
- 7 a.m. to 4:30 p.m., Thursday
Health Science Building, north entrance
- 6:30 a.m. to 7:30 p.m., Monday through Thursday
Applied Technology Center, north entrance off Stone St.
- 7 a.m. to 7:30 p.m., Monday through Thursday
- 7 a.m. to 4:30 p.m., Friday
Clara E. Mackenzie Building, south entrance
- 7 a.m. to 7 p.m., Monday through Thursday
- 7 a.m. to 3 p.m., Friday
Screening procedure
Complete the online COVID-19 Self-Assessment Form and, if you are cleared to be on campus, keep the green checkmark “Cleared” screen visible on your phone, including the date shown under the checkmark. You may also show the confirmation email you receive or print out the confirmation and show the paper copy to the screener.
Proceed directly to one of the screening locations after parking or upon entering campus property. Show your green checkmark confirmation screen (or the paper copy) to the screener, who will give you a wristband. You are responsible for putting on and wearing your wristband. Once you have your wristband you may enter any campus building for the rest of that day. A different colored wristband is issued each day.
All individuals must wear a face mask during their entire time on campus, including during this screening period. You must be wearing the correct color wristband for that day to enter any campus buildings.
If you do not feel well and/or do not pass the self-screen, please stay home.
